A single-member LLC owned by a non-resident must file a Form 5472 together with a pro forma 1120 every year, including with no income and no activity; the deadline is April 15 and the IRS sets a penalty of up to $25,000 per missing or incomplete form. A multi-member LLC files a Form 1065 with its K-1 schedules instead, by March 15. What exactly is Form 5472?

Form 5472 reports to the IRS the transactions between your LLC and yourself, its foreign member: contributions, withdrawals, loans, invoicing. For a single-member LLC it is never filed alone — it goes with a pro forma 1120, a carrier form whose only job is to carry it.

It is an information return, not a tax assessment: it informs the U.S. authorities, it calculates no tax.
